Wondering if ADHD could be part of your story? Our evaluation process is designed to give you real answers in a thoughtful, step-by-step way.

  • Step 1: Intake & Screening

After booking, you'll receive an intake packet that includes:

    • Consent forms

    • ADHD screening and assessment questionnaires

    • Questions about your medical and mental health history

This step helps your provider determine if you're a good fit for care at Kiwi. We carefully review your responses before confirming your appointment. If anything in your intake indicates that you'd be better served in person, we’ll cancel the appointment and issue a full refund.

    Step 2: Comprehensive Evaluation

Once your intake is reviewed, you'll meet with your provider for a deep-dive discussion. This session includes:

    • A closer look at your history and symptoms

    • An exploration of how you’ve managed or masked symptoms over time

    • Consideration of things like your strengths, environment, upbringing, and possible coexisting conditions (like anxiety or depression)

Step 3: Diagnosis & Treatment Planning

If ADHD is diagnosed, we’ll talk through treatment options and tailor a plan that fits your needs and goals—whether that includes medication, lifestyle changes, or both.

Please note: All patients must complete this full evaluation—even if you’ve already been diagnosed or treated for ADHD elsewhere.

  • Each visit is a check-in on how you’re feeling, what’s working, and what needs adjusting
      • We’ll review symptoms, progress, and any lifestyle changes
      • Medication adjustments are made collaboratively

  • Safety is a priority.
      • We follow all DEA standards for prescribing controlled substances
      • That includes reviewing your prescription history (PDMP), as required by state law, watching for problematic behaviors, and occasionally requesting a urine drug screen

  • Support goes beyond medication.
      • We offer lifestyle strategies to help you manage ADHD in real life
